In a major step toward digital transformation in healthcare, the UK government has announced the development of a world-first AI-powered early warning system designed to detect patient safety issues across the National Health Service (NHS) in real time. The initiative is part of the government’s broader 10 Year Health Plan and aims to trigger faster inspections and prevent patient harm before it occurs.

Unveiled by Health and Social Care Secretary Wes Streeting, the AI system will scan routine hospital data and staff reports to automatically flag safety concerns—such as patterns of abuse, injuries, or unexpected outcomes—that may otherwise go unnoticed. The Care Quality Commission (CQC) will use the alerts to deploy specialist inspection teams rapidly when risks are identified.

The system’s first application begins in November 2025, with the rollout of a maternity outcomes signal system across NHS trusts. It will monitor for elevated rates of stillbirth, neonatal death, and brain injury, using near real-time data.

“This technology will save lives—catching unsafe care before it becomes a tragedy,” Streeting said. “Patient safety and power are at the heart of our 10 Year Health Plan.”

Built on the NHS Federated Data Platform, the new system enables healthcare staff to securely access and act on data from a single source. That means less manual oversight and more time dedicated to patient care.

The plan follows mounting pressure to improve safety in the wake of high-profile failures in mental health and maternity services. CQC Chief Executive Sir Julian Hartley noted that the AI system will help regulators spot inequalities in access and outcomes earlier.

The move marks the NHS in England as the first health system globally to adopt an AI-enabled warning network, reinforcing the UK’s position as a leader in digital health innovation.
